Notice:USCG PHILADELPHIA COTP ENTRY REQUIREMENTS***

DUE TO HEIGHTENED SECURITY CONCERNS THE USCG ORDERS THAT ALL VESSELS
DESIRING ENTRY TO THE CAPTAIN OF THE PORT PHILADELPHIA ZONE REMAIN OUTSIDE
THE THREE NAUTICAL MILE LINE UNTIL GIVEN WRITTEN AUTHORIZATION BY THE
CAPTAIN OF THE PORT TO PROCEED INBOUND.

USCG ALSO REQUIRES THE VESSELS TO PROVIDE INFORMATION VIA FAX TO 215-271-
4899

1. SUBMIT COPIES OF
SAFE MANNING CERTIFICATE
CURRENT CREW LIST
DANGEROUS CARGO MANIFEST IF APPLIES

2. VESSEL MASTER AND C/E TO SUBMIT WRITTEN REPORT OF EXAMINATION OF
VESSELS HIGH RISK AREAS FOR ANY INDICATION OF UNUSUAL ACTIVITY OR EVIDENCE
OF TAMPERING

3. HAVE ANY CONFINED SPACES ENTRY PERMITS BEEN ISSUED DURING THIS VOYAGE?
PROVIDE COPIES OF EACH WITH A DESCRIPTION OF THE WORK PERFORMED

4. THE MASTER SHALL THOROUGHLY EXAMINE ALL CREW LICENSES AND DOCUMENTS
AND VERIFY VALIDITY

PROVIDE INFORMATION AS FOLLOWS:

ISSSUING FLAG AUTHORITY FOR ALL LICENSES/DOCUMENTS HELD BY EACH INDIVIDUAL
WHETHER TO LICENSE/DOCUMENT IS PERMANENT OR TEMPORARY

5. IDENTIFY FOR EACH CREWMEMBER, IF NOT ON CREWLIST

WHEN DID THE CREWMEMBER JOIN THE VSL?
IN WHAT PORT DID THE CREWMEMBER JOIN
HAS THE CREWMEMBER HAD PRIOR EMPLOYMENT ABOARD THIS VESSEL OR WITH THE
VESSEL OWNER/OPERATOR

6. HAVE ANY SUSPICIOUS/UNUSUAL ACTIVITIES BEEN OBSERVED ABOARD THE
VESSEL, IF SO DESCRIBE

7. ARE THER ANY PASSENGERS/SUPERNUMERARIES ON BOARD, IF SO LIST ON
CREWLIST AND SPECCIFICALLY STATE THE REASON FOR THE INDIVIDUALS TO BE ON
BOARD AND PORT OF EMBARKATION


FOR PASS. VESSELS ONLY--
8. PROVIDE COPY OF PASS. LIST INCLUDING NATIONALITY EACH PASSENGER
A PASS./CREW MUSTER SHALL BE COMPLETED AND RESULTS PROVIDED. THE MUSTER
SHALL PROVIDE FOR A FULL ACCOUNTABILITY FOR ALL PERSONS ONBOARD SINCE THE
LAST PORT VISIT

CONFIRMATION THAT THE SECURITY MEASURES REQUIRED FOR A MEDIUM THREAT HAVE
BEEN MET
